Rambam Family Health’s Care Management Department provides 360º care for patients to improve health outcomes. Our primary and specialty care providers and care coordinators work together to ensure continuity of care outside of your regularly scheduled visits, and coordinate with providers outside of Rambam. Our care management team will check in with you regularly to see if there have been any changes you wish to discuss or have any medical needs that should be addressed (including helping you obtain supplies or services you may need to manage chronic conditions).
